Risk Management

Each year WASA coaches must fill out a Risk Management Form.  This form allows the Ohio South Youth Soccer Association (OSYSA) to perform background checks on randomly selected coaches, administrators and other volunteers.

To fill out the Risk Management Form:

  1. Go to the form
  2. Fill out all required information
  3. In the Member League Code field, select "Westerville Amateur Soccer"
  4. Press the Submit button at the bottom of the form
  5. Print 2 copies of the certificate that indicates that you have submitted the Risk Management form. Give your league coordinator 1 of the certificates and keep the other for your records.
